You may not say to others that your wrinkles were developing gradually and then suddenly. What are Dermal Fillers?

There are several different types of injectable treatments that are performed today. Not all injectables that are made for wrinkle-reduction fall into the category of dermal fillers. These products, such as Juvederm and Restylane, are wrinkle-reducers and then some. Unlike neuromodulators that soften dynamic wrinkles by targeting muscle tissue, dermal fillers smooth lines and creases by adding volume to deeper layers of the skin. This is beneficial because volume loss is one of the critical biological factors that stimulate the onset of creases like nasolabial folds and marionette lines, as well as jowls, thin lips, and flat cheeks. The fillers we’ve mentioned achieve their beneficial effects by depositing hyaluronic acid into layers of the skin. The particles of hyaluronic acid trap water molecules right where they are needed to contour the designated area. Furthermore, there are several products within these lines that have been specially made to address specific problems. More structural fillers are used in the cheeks and jawline area. Softer fillers may be used around the eyes or on the lips. This allows the provider to achieve the most natural-looking results for each patient. Sculptra Helps You Age More Gracefully. Here’s How!

istock 1193929408 1 Each of us ages in our own way. The way we feel about aging is also something that differs from one person to another. Therefore, we need to have options that allow us to address the signs of aging in ways that feel good to each of us. Some people prefer to undergo plastic surgery to achieve the most long-lasting and dramatic results, while others like the idea of injectables and other nonsurgical treatments that are subtler. We love that we have the opportunity to assist our patients based on their preferences and needs. Sculptra is one tool that we may use to do that.

What is Sculptra?

Sculptra is described as an FDA-approved dermal filler, but we think that misses the mark. Sculptra is an injectable treatment, yes, but it does not work the same way most dermal fillers do. Dermal fillers are made with tiny particles that immediately build volume beneath the skin. Some do this with hyaluronic acid, which binds to water to smooth creases and contour areas of the face. Some build volume with calcium microspheres. Sculptra doesn’t build volume in such a direct way; it does so by stimulating ongoing collagen production.

Collagen stimulation is necessary as we get older because the body slows down its natural production to a near-stop. By injecting Sculptra’s poly-L-lactic acid particles beneath the skin and fat pads on the face, low-level inflammation is created. This forces fibroblast cells to activate and make new collagen to support the inflamed tissue. This makes the skin thicker, firmer, and smoother.

The Pros and Cons of Sculptra Aesthetic


  • Creating change through collagen stimulation, Sculptra can improve the appearance of the aging face for 2 or more years at a time. Collagen naturally degrades with time but not in the same way that hyaluronic acid does. Therefore, Sculptra typically achieves longer-lasting results than the average dermal filler.
  • After the initial series of treatments, patients may need less product during subsequent touch-ups years later. This depends on how quickly collagen breakdown occurs based on genetics, sun exposure, and other factors.
  • Collagen stimulation occurs for several months, so the results of Sculptra are gradual, like aging backward.


  • The cost of Sculptra may seem higher than dermal filler treatment. However, the two treatments are not the same so it is difficult to compare them. In the long run, cost-effectiveness may be very similar.
  • Patients must understand that Sculptra does not provide them with immediate, dramatic results. The skin is rejuvenated over time as it is supported by healthier, stronger collagen fibers.
  • A series of approximately 3 treatments may be needed to achieve the most significant collagen stimulation. These are usually scheduled six weeks apart.

Variations in Dermal Fillers That You May Not Know

Dermal fillers represent one of the most popular beauty treatments to exist in our lifetime. A quick visit to a dermatologist or plastic surgeon is all it takes to smooth stubborn lines and creases, plump the lips, and even rejuvenate the backs of the hands. Because the benefits of dermal fillers are well-known, the interest in injectable treatment continues to increase among adults of all ages. Here, we point out how two fillers, Sculptra and Radiesse, are not like others.

What dermal fillers are known for is adding volume to the skin. It is the increase in volume that brings wrinkles to a smooth surface. It is volume increase that makes the lips look fuller and younger. The most well-known dermal fillers are made with a sugar compound, hyaluronic acid, that increases volume by binding to water molecules. In the case of Sculptra and Radiesse, the objective is almost entirely different.


Sculptra is placed into the category of dermal filler because there is currently no other place for it. This injectable is made of the same synthetic material as dissolvable stitches, poly-L-lactic acid. The fact that poly-L-lactic acid has a long history in medicine can give patients peace of mind about its safety and efficacy. As a dermal filler, poly-L-lactic acid exists as a dense liquid. When it is introduced into the skin, it creates a minor inflammatory response. This is the beginning of collagen production. Collagen is the protein that forms the skin’s foundation and keeps skin firm and resilient. Over time, new collagen forms around the poly-L-lactic acid that has been injected, restoring more youthfulness, like aging backward. It does take time for the effects of Sculptra treatment to emerge. When they do, they last about 2 years.


Radiesse is both a collagen-stimulator and a dermal filler. This product is formulated with tiny particles of calcium hydroxylapatite. This is similar to the calcium found in bones and teeth. The calcium hydroxylapatite microspheres initially act as a new scaffold beneath the skin, plumping tissue like a traditional dermal filler. Additionally, this substance also triggers a slight inflammatory response that brings more collagen in to surround the microspheres and to stay in the area once the calcium hydroxylapatite has dissolved. The results of Radiesse treatment may last approximately 12 months.

Each, though, uses a proprietary technique of cross-linking to achieve unique smoothness and cohesiveness in various products. Cross-linking is the technique of stringing hyaluronic acid particles together so they are dissolved more slowly. Products like Juvéderm Vollure utilize additional techniques, such as Vycross technology, to refine the molecular weight of the filler. This translates into improved comfort and less swelling.

Juvéderm Voluma & Restylane® Lyft

Both made of hyaluronic acid and both designed for the correction of mid-facial volume loss, Voluma and Lyft are created with more cohesiveness. Their gel consistency is thicker because the intention is to reinstate the structure of the cheekbones through deep insertion beneath the skin. These denser filler products have optimal moldability and integrity that sustain results for up to 2 years.

Juvéderm Volbella & Restylane® Silk

Dermal fillers are not reserved only for lines and wrinkles. Products like Volbella and Silk are frequently used to enhance the lips. These fillers are made with small particles of hyaluronic acid in a smooth, fluid gel that inserts seamlessly beneath the skin. They are dense enough to subtly increase volume in thin and deflated lips and silky enough to smooth the fine lines that form around the mouth.

According to the American Society of Plastic Surgeons, well over 10 million nonsurgical procedures are performed annually. Of them, injectables like dermal fillers are among the most common. Evidence of dermal filler treatment can be found with a single scroll through social media or a quick browse of a magazine. This has taken the mystery out of filler treatment. At the same time, the popularity of plumped lips and smooth skin may lead to the idea that fillers can accomplish just about anything. It is important to know both the capabilities and the limitations of this nonsurgical treatment process, including how much filler may be needed for your unique concerns.

How Fillers Work

The objective of dermal filler treatment is to increase volume in the dermis. This is achieved by inserting a substance such as hyaluronic acid into the skin. In this instance, the inserted substance binds to water molecules. It is the trapped water that supplies volume for younger-looking facial contours. What determines how much volume is achieved is the number of water molecules that can be tethered. However, the amount of volume that is needed may vary from person to person, and this is where we begin to see how much dermal filler may be necessary.

Factors that Contribute to Filler Treatment

Facial aging is not limited to changes in the skin. In addition to losing resilience in the dermis and epidermis, we all will experience some degree of bone loss, too. In the face, this loss tends to occur around the eyes. Imaging demonstrates that, with age, the sockets around the eyes grow larger, allowing the eyes to sink back slightly (or significantly). The extent of this loss will affect how much filler a person may need to restore a more youthful appearance. Additionally,

  • Skin tone that is lax and weak may be more challenging to correct. Fillers cannot tighten the skin; these products only smooth it by adding volume. When the skin is very loose and wrinkled, more filler may be needed. Too much filler, though, creates a puffy appearance. If laxity is an issue, laser treatment like Halo resurfacing may be an excellent compliment to dermal fillers.
  • Fat also affects the outcome of filler treatment. When there is very little fat on the face, it takes more filler to add contours.
